What do I need to do to get started with care?

HomeCare Transitions can start by visiting you and your loved one in your home, with the discharge planner from the facility or hospital to discuss available options. This ensures that your care plan is designed with your specific needs and budget in mind.

How much does home care cost?

HomeCare Transitions ALWAYS works with family to develop care that is flexible and affordable. We base our rates solely on the type and amount of care that is actually needed at that time. As care needs increase or decrease, the rate may move accordingly.

Does HomeCare Transitions accept insurance?

We work with many insurance carriers that provide coverage for home care. We will work with your insurance provider to determine if coverage is available and what the level of coverage may be.

Does HomeCare Transitions provide services outside of Battle Creek?

YES. We provide care throughout all of Southwest Michigan, from Jackson County to St. Joseph north to Kent and Ingham County and south to the state line.

Does HomeCare Transitions work with the Veterans Administration?

YES. HomeCare Transitions are part of the Veteran's Choice Program. If you are already enrolled in VA health care, the Choice Program allows you to receive health care within your community. Using this program does NOT impact your existing VA health care, or any other VA benefit. And, you may request HomeCare Transitions to be your provider.
